
Survey for Children and Young People with Special Educational Needs and/or Disabilities (SEND)
This survey is for children and young people with Special Educational Needs and Disabilities (SEND).
It is aimed at children and young people aged 8 to 25, however we welcome responses from younger children too.
You may want to fill this survey out with help from your parent, carer or teacher. All the questions are optional so you can skip a question if you don’t want to answer it.
We want to make sure that children and young people with SEND get all the help they need. This is your chance to tell us about the help you get with your learning, your health and planning your future.
After the survey has closed, we will put a summary of the information we receive on the survey page and the SEND Local Offer. We will not share any personal details. This information will tell Lambeth Council what is working well and what we need to do make the lives of children and young people with SEND better.
This survey is anonymous – this means no one will know who said what. Please don’t write your name or anything that tells us who you are.
If you are under 16, please make sure your parent, carer or teacher knows you are doing this survey and is happy for you to take part.
If the survey is completed in school, schools are asked to follow their usual consent and safeguarding procedures.
