Reach for the Stars 2025 Nomination Form
Celebrating the Achievements of Lambeth’s Young People in Care and Care Leavers
Join us in celebrating the achievements of looked after young people and care leavers aged 13 to 25. Reach for the Stars Awards highlights their resilience, talents, and accomplishments.
Who Can Nominate? Social workers, Personal advisors and Independent Reviewing officers can nominate the young people they work with.
Nomination Categories:
Education Achievement.
Examples: Punctuality and attendance, improved grades, perseverance, resilience, or participation in extracurricular activities.
Dedication and Excellence.
Examples: Mastering a skill or hobby, overcoming obstacles, going the extra mile or showing dedication.
Leadership Achievement.
Examples: Shown resilience and strength, supports others, has leadership qualities or set up a social enterprise.
Participation and Personal Development.
Examples: Active in meetings, contributing at home, volunteering, developing life skills, or engaging in hobbies.
Personal Resilience and Determination.
Examples: Overcame fears or anxiety, managing with health challenges, improving their self-esteem, or demonstrating determination.
Key Details:
- Nominees must be aged 13 to 25.
- A separate ceremony will celebrate those aged 12 and under.
- Nominations are required for young people to receive an invitation.
- Young people can only be nominated in ONE category.
- Each nominee can bring one guest (or two for young people with support needs).
- Ensure all information is completed.
